Types of the official document “Declaration of Private Interest”
The official document “Declaration of Private Interest” is approved with “Order no.77″ dated 26.12.2008 of Inspector General of HIDAA “An amendment on order no.36 dated 10.10.2006 of Inspector General for “The approval of the types of private interest declaration form”.
Below you can find the four types of declaration form in “word” format, which you can complete, print, sign and submit in a closed envelope to the Representative Authority of the institution where you are exercising your public function.
